Our PCA/CFSS Services

What we do

We offer a wide range of high quality care services for our clients from PCA services to 245D waiver service. Here are some of the services we offer:

PCA services provide assistance and support for persons with disabilities, living independently in the community. This includes the elderly and others with special health care needs. PCA services are provided in the Minnesota Health Care Programs (MHCP) member’s home or in the community when normal life activities take him or her outside the home.

Personal Care Assistance (PCA)

Community First Services and Supports (CFSS) is a Minnesota health care program that will offer flexible options to meet the unique needs of people. CFSS allows people greater independence in their homes and communities. CFSS will replace personal care assistance Personal Service Assistance (PCA), and Community Support Grant (CSG).

Community First Services & Support (CFSS)

In Minnesota, "245D Services" refer to Home and Community-Based Services (HCBS) These services are for individuals with disabilities and those over age 65, provided through Medicaid waiver programs to support independence and community integration as an alternative to institutional care. We are 245D licensed Provider for both basic and intensive
